In order to apply for employment all new applicants MUST bring the following. You will not be permitted to apply without these required documents:
NYS HHA/PCA Certificate (Original Only)
Recent In-service Certificate
Two Recent Letters of Reference
(Must be from employers and include phone numbers)
Current Annual Physical Exam (Request Jos-El Care Physical Exam form)
Annual Tuberculosis Test (One of the following)
-Mantoux Skin Test (PPD) OR
-Quantiferon Gold (QFT) OR
-Chest X-Ray (Hx of Positive PPD/QFT
Proof of immunity (Titers w/ exact date (mm/dd/yy) and numerical values) for:
- Rubella Titer
- Rubeola Titer
- Varicella Titer
Drug Screen
Tetanus/Diphtheria Vaccine/Titer (Td/Tdap/DPT) (within 10 years) OPTIONAL
Two (2) Photo Identification (NYS ID, Driver License, Passport, Permanent Resident Card and/or Work Authorization Card.)
Social Security Card (Original Only)
